New Google forms available:

We have created Google forms to accommodate your HSIP request extension and card return requests. You may access the forms at the links below, and you will receive separate confirmation from the HSIP team, letting you know we have processed an extension or that you may return cards. You may always send an e-mail (subject-incentives@umich.edu) to accomplish these tasks, if you do not wish to use the forms.

HSIP System Release (August 19, 2014) - Known Issues:

An HSIP system release was moved to the production environment on Tuesday, August 19, 2014. Several unintended issues arose as a result of the migration, and we are working with ITS to fix them as soon as possible.

As of October 23, 2014, the known issues are:

Open items:

  • Approvers are unable to view attachments they upload.
    • Status - ITS is investigating.

Resolved items:

  • Individuals listed as Alternate Approvers are not able to access an HSIP request on which they are an Alternate Approver.
    • Status - Resolved.
  • Total Coupon Amount error (25712,89). The error is meant to alert the requestor that the Total Incentive Amount in the HSIP request is less than the number of coupons requested times the maximum amount to be paid per subject. The system should allow the request to be submitted after generating the alert, but is not.
    • Status - Resolved.
  • HSIP requests listed more than once in the HSIP Home Page.
    • Status - Resolved.
  • Delivery information (address, city, state, etc.) fields disappear when requesting a Check payment sent to the Researcher.
    • Status - Resolved.
  • "Files Attached" checkbox is active (i.e., checked) even when no attachments are present in the request form.
    • Status - Resolved.
  • Individuals listed as Alternate Custodians are not able to access an HSIP request on which they are listed as an Alternate Custodian. This functionality allows alternate custodians to attach documents to the HSIP request form.
    • Status - Resolved.

If you would like to sign up for either a classroom session or an individual training session, please use the Google form. You will receive a separate confirmation from the HSIP office, indicating that you are registered for the selected training session.  

Bank Holidays:

When there is a University of Michigan or bank holiday, the Treasurer’s Office will not be running any checks. On the following holidays there will be no check run. Please make sure to avoid requesting checks on these dates. Any checks requested on these dates will be moved to the following business day.
