University of Michigan Health System Mcards
All persons affiliated with the U-M Health System (UMHS) are required to wear a current Health System Mcard. UMHS Mcards are only issued at the Key & ID Office in the Med Inn Building. If a person requires access to the Health System and Ann Arbor, Dearborn, or Flint campuses, the cardholder must still obtain the Mcard at the Key & ID Office. UMHS Mcards can provide access to Health System and campus buildings (even with smart card building readers). However, campus Mcards do not provide access in Health System buildings.
The U-M Health System Mcard Request and Change form is required for all new and replacement Mcards with the exception of damaged Mcards. The Key & ID Office does not accept card or replacement fee payments. Payments must be made at the Hospital Cashier's Office. The paid receipt is then given to the Key & ID Office as proof of payment.
Government photo ID (i.e., driver's license, state ID, passport) is required at new Mcard issuance. People wearing sunglasses, hats, caps, bandanas, etc. will be required to remove them before their photo is taken.
Students, Faculty, Staff, and U-M Temps Staff Services
People who have a previous Mcard will be required to turn it in to obtain the new Mcard. Student, faculty, and staff Mcards are valid up to five years, while the U-M Staffing Services Temps are valid up to one year. The University department or unit name will print under cardholder's name. Expired Mcards and new Health System Mcard Request and Change form are required to be turned in to the Key & ID Office for new Mcards.
Contractor, Un-Paid Staff, Vendor, Visitor, & Volunteer Mcards
These cardholders are required to bring a completed Health System Mcard Request and Change form to Key & ID Office and a paid receipt from the Hospital Cashier's Office for $6.00 for an Mcard. Some Health System units sponsor the $6.00 costs with internal billings.
People who have a previous Mcard will be required to turn it in to obtain the new Mcard, otherwise a replacement fee may apply. Mcards are valid up to one year. The University department or unit name will print under cardholder's name. Expired Mcards, new Health System Mcard Request and Change form, and paid receipts are required to be turned in to the Key & ID Office for new Mcards.
