Ann Arbor Campus departments and units can complete this form for new Ann Arbor faculty, staff, and others not affiliated with the U-M Health System to request their first Mcard or for cardholder who is transferring to a new department.  Students and alumni do not need to complete this form.

It is generally not required for replacement Mcards (lost, stolen, expired, or personal name changes). 

It is required for the replacement of expired cards for contractors, temporary employees, unpaid staff, vendors, and visitors.  Government photo ID (driver's license, state ID, or passport) will also be required at the time of Mcard issuance. Please read form instructions for additional details.

Individuals requiring a U-M Health System, North Campus Research Complex (NCRC), Dearborn or Flint Mcard must not utilize this Ann Arbor form.  Please select that areas' specific form.
