The request form for New Endowment or Quasi-Endowments has been updated to make it more user-friendly as well as to provide a cohesive look with the other Accounting Services' forms. The new form can be found here: http://www.finops.umich.edu/accounting/forms/endowmentrequest. To ensure that you always have the most recent version of the form, please bookmark this page for future use.The updates to the form include:

• More defined, user-friendly sections: Request Type, Contact Information, Endowment Information, Department Approval, Financial Operations Use, and Processing
• The Processing section now includes the two ways to send the request form back to Accounting Services: via e-mail or campus mail
• An internal use section for Financial Operations
• The inclusion of the Entity ID (EID) for imaging purposes
